A DOT physical is a medical qualification examination for drivers subject to applicable commercial motor vehicle requirements. It evaluates health factors relevant to safe driving. It is not a driving test, and completing it does not itself issue a CDL.

The exact requirements depend on the driver's situation and applicable rules. Use current FMCSA and state licensing information alongside the examiner's instructions rather than relying on an old checklist.

Choose the appropriate examiner

For an examination subject to FMCSA requirements, use a clinician currently listed on the National Registry of Certified Medical Examiners. Confirm the examiner and appointment type before booking. A routine annual physical or TLC examination is not automatically a substitute.

Tell the office if your employer has supplied forms or additional testing instructions. This helps identify services that are separate from the medical qualification examination.

What the examination considers

The examiner reviews health history, medicines, and relevant conditions and performs the required physical assessment. This includes areas such as vision, hearing, blood pressure, and urine testing. The findings and any additional records help the examiner make a certification decision.

A urine test performed as part of the examination should not be assumed to be a drug test. If your employer requires drug screening, ask about that requirement separately.

Prepare your documents

  • Bring identification and relevant current examination or certification paperwork.
  • Bring glasses, contact lenses, or hearing aids that you use.
  • Prepare an accurate list of medicines and treating clinicians.
  • Bring requested records for existing conditions and previous evaluations.

Ask the examiner's office in advance which records are needed. Do not stop medicines, change doses, or try to conceal a condition to influence the examination. Accurate information supports a valid assessment.

Understand possible next steps

The examiner may issue a certificate, request more information, or determine that requirements are not met. Certification length and follow-up depend on the findings and applicable rules. Neither an appointment fee nor a previous certificate guarantees the next result.

Ask how certification information will be communicated and what you need to do under your state's current process. Do not assume that receiving a paper card completes every licensing step.

Questions to ask before leaving

  • Is more information needed to complete the assessment?
  • What is the certification decision and any follow-up requirement?
  • What documents will I receive?
  • What must I confirm with my employer or licensing authority?
