About CDL / DOT Physicals

Health Med provides DOT physical appointments for commercial drivers in Jamaica, Queens. A CDL medical exam evaluates whether a driver meets the applicable medical qualification requirements. It is separate from a driving test and does not itself issue or renew a commercial driver's license.

Book the correct examination

Tell our office that you need a DOT physical or CDL medical certification when arranging your visit. For examinations subject to FMCSA requirements, confirm that the examining clinician is currently listed on the National Registry of Certified Medical Examiners. Ask about the exam fee and any additional testing before your appointment.

A TLC medical exam is a different service with different forms. If you drive a taxi or for-hire vehicle, check which examination your licensing authority has requested before booking.

What the visit involves

The examiner reviews your health history and medicines and carries out the required examination. This includes checks such as vision, hearing, blood pressure, and urine testing. A urine test during a physical is not automatically the same as an employer-required drug test; confirm any separate testing requirement with the office.

The examiner determines whether certification can be issued and whether additional records or evaluation are needed. The result, duration of certification, and completion time depend on the findings and applicable requirements. An appointment is not a guarantee of certification.

What to bring

• Your identification and any current medical certificate or examination paperwork.
• Glasses, contact lenses, or hearing aids that you use.
• A complete medication list and relevant records from treating clinicians.
• Any forms or instructions supplied by your employer or licensing authority.

Ask in advance if the examiner needs records relating to an existing condition. Do not stop prescribed medicines or change treatment to prepare for the examination unless your treating clinician instructs you to do so.
