Health Med provides job physical appointments for people completing employer-requested health examinations. Bring the employer's instructions so our office can confirm the type of appointment, required forms, and any additional testing before you visit.

An employment physical is tailored to the request. There is no single examination package that meets every employer's requirements, and a general physical is not automatically a DOT examination or a drug screening appointment.

Start with your employer's paperwork

Ask your employer for the correct form, a description of the required examination or tests, and the deadline. If a specific testing method, certification, or reporting process is required, share that information with the office in advance.

Confirm who is responsible for payment and where completed documents should be sent. If you are arranging appointments for a group, contact the office about scheduling and the services required rather than assuming the same process applies to every employee.

What your appointment may include

The clinician reviews the relevant medical history and performs the requested physical assessment. Depending on the employer's requirements, additional checks or laboratory testing may be necessary. The office should confirm what can be completed at the center and what may require a separate appointment or outside provider.

Results and completion times depend on the examination and any outstanding tests. The clinician documents the findings; the visit is not a promise of employment or an automatic clearance for every job.

• Photo identification and the employer's current forms.
• A list of medicines and relevant medical records.
• Glasses or hearing aids used for work or daily activities.
• Employer billing information, if applicable, and your insurance details.

Ask whether any preparation is needed. Do not fast or change prescribed medicine unless you receive specific instructions from a qualified clinician.
