The most useful first step before a school or sports physical is obtaining the correct form. Schools, camps, and athletic programs may ask for different information. A form found online is not necessarily the one your child's program accepts.

Use this checklist to organize the appointment and avoid missing paperwork. The examination itself and any participation decision remain the responsibility of the evaluating clinician.

Get instructions from the school or program

Ask for the current form, submission deadline, and instructions about how recently the examination must have been completed. Check whether the request is for general school health documentation, sports participation, or another purpose.

If the instructions require a particular examination or additional document, share them with the clinic before booking. Do not assume that an old form can simply be signed again without review.

Complete the sections intended for you

Fill in parent, guardian, and student information where requested. Answer health-history questions accurately, including previous injuries, medicines, allergies, and relevant symptoms. Leave clinician sections for the healthcare professional.

If a question is unclear, mark it for discussion rather than guessing. Keep a copy of the form and the program's contact details in case clarification is needed.

Gather supporting records

  • Immunization records and any requested school health documentation.
  • A current medication list and information about allergies.
  • Relevant reports from previous injuries or specialist assessments.
  • Glasses, contact lenses, or other aids your child uses.
  • Insurance information and required parent or guardian identification.

Tell the clinician about symptoms during activity, changes since the last examination, and any existing care plans. The purpose is to make participation as appropriate and informed as possible, not only to obtain a signature.

Allow time for follow-up

Book before the deadline when possible. Missing records or a finding that needs further evaluation can affect when a form is completed. A clinic cannot promise clearance before the examination has taken place.

Ask how you will receive the completed document and whether the clinic or family submits it. Check that the necessary sections are completed, keep a copy, and follow the school's submission instructions.

Keep the visit focused

Employment, TLC, DOT, and immigration forms have different purposes. They should not be treated as interchangeable with a school physical form. If more than one examination is needed, explain this when scheduling.

Health Med's school physical service in Jamaica, Queens, provides appointment information. Contact the office with your child's age and the exact form so it can confirm whether the requested visit can be arranged.
